Have you ever felt like your thoughts were trying to move through thick soup? You're reading, but the words just won't stick. You start a sentence and forget where you were going. That, in a nutshell, is brain fog, and it’s an incredibly common and frustrating symptom for many people living with Postural Orthostatic Tachycardia Syndrome (POTS).


But why? Why does a condition often talked about in terms of heart rate and blood pressure affect your concentration and memory? The answers are actually quite fascinating and offer real clues into how to manage it.


Your Brain on Empty?

The biggest player is almost certainly blood flow. Let’s think about how POTS works for a moment. When a person with POTS stands up, for reasons not perfectly understood (often linked to the nervous system not signal tightening blood vessels properly), gravity pulls too much blood into the legs and tummy area. 


The body responds by making the heart beat very fast to try and pump blood upward, but it’s still not always enough to properly perfuse – fancy word for feed – the brain.


Our brains are total energy hogs. Even though they are relatively small, they consume a massive amount of the body's total oxygen and glucose. If that steady supply is even slightly compromised by poor circulation, things start to slow down. Think of it like a computer struggling with low power; it might still run, but all the processes drag. This temporary dip in oxygen and blood flow is a major, and very logical, reason for the sudden fuzziness many experience, especially right after standing.


The Signal Scramble


It's not just about what flows to your brain; it’s also about the instructions the brain sends out. POTS is, at its core, a disorder of the autonomic nervous system. This is the background system that handles things you don’t think about, like digestion, heart rate, and temperature control. In many folks with POTS, this system is a bit out of whack, sometimes described as having a hyper-responsive ‘fight or flight’ system and a slightly underactive ‘rest and digest’ one.

Inflammation and Other Factors


The story doesn’t stop there. Research is starting to show that inflammation might also play a role in POTS brain fog. There’s increasing thought that for a subset of POTS patients, especially those whose symptoms started after a virus, an underlying auto-immune or low-level inflammatory process could be at play. When the body is slightly inflamed, it can directly mess with brain chemistry and connection, furthering the feeling of being in a fog.


And of course, we can’t forget the other fun parts of living with POTS that directly impact the brain. Many people struggle with sleep disturbances because of heart rate spikes or other issues at night. Being chronically sleep-deprived is guaranteed to cause mental slowness in anyone. Add in any associated factors like chronic pain or nutritional imbalances that sometimes accompany POTS, and it’s no wonder clarity is so hard to find.
